Please fill out the documents to the best of your ability and bring them to your evaluation and treatment appointment. In addition to the forms below, please bring the following to your first visit:

1. Your driver’s license or photo identification.

2. Your insurance card(s) or letter of authorization for your worker’s compensation claim.

3. Colorado is a direct access state - we can check your insurance company’s policy to determine whether you will need a prescription from your doctor for Physical Therapy services. A referral or prescription from your doctor will be necessary if you are billing through Medicare.  

4. Your calendar.  We have a list of patients waiting to begin their therapy with us and would like to schedule as many of your appointments as possible at your first visit to avoid scheduling conflicts and to ensure you have the best outcome from physical therapy.

5. You may choose to download and print the Intake Paperwork if you have a printer. Otherwise, please arrive 15 minutes before your first scheduled appointment to complete the required forms (see attachment below).

We have insurance contracts with most major carriers, including Medicare. Our cash rates are $85 for a one-on-one 40-minute evaluation/treatment session with a physical therapist.
